Can't say what happened or what caused it.
OOB's can happen for a variety of reasons, often times slam fires before the bolt is locked. Meaning, the primer gets set off before the trigger is pulled. Can be caused by all sorts of things - firing pin breaking, primer too soft or corroded or some damage, bad or excessive headspace. OOB's can also be caused by mechanical failure. Like if you break a lug off, or other true mechanical failures - some would make abnormal effects, like I'm not sure what would happen if your forward trunnion broke on an
AK. Probably be bad..
Case failures are also possible, resulting in a lot of gas blowback (maybe even enough to blow the cover off as shown) but they are not quite as bad so long as the bolt is locked - at least some pressure is expended out the barrel. May have even been what happened here, I can't say by looking at it.
I've only ever had one
OOB in my career with factory ammo and it basically blew the rifle apart. Info on that is in another thread (carbine) so I won't relate it here.
